Walker Healthforce is seeking an Social Worker II for a healthcare client located in Oklahoma City, OK. This is a contract position.
START DATE: ASAP

JOB DESCRIPTION
- Provide social services and psychosocial assessments for assigned medical service areas
- Conduct discharge planning evaluations and collaborate with hospital staff
- Arrange placement into LTACs, SNFs, home health services, or community programs
- Coordinate and order DME (durable medical equipment) and ensure patients' post-hospital needs are met
- Engage DHS/APS for cases requiring social intervention or protective services
- Provide crisis counseling for cases involving abuse, neglect, or traumatic diagnoses
- Advocate for patients in ethical decisions and end-of-life care situations
- Counsel patients and families adjusting to illness, treatment, or functional decline
JOB REQUIREMENTS
- Inpatient hospital discharge planning experience – including LTAC/SNF placement, DME ordering, home health coordination, and working with DHS/APS
- Master’s Degree in Social Work from an accredited institution
- Strong knowledge of social and psychological casework principles and community/social service resources
- Ability to evaluate patient situations and make informed care decisions
- Strong interpersonal, advocacy, and communication skills
- Capable of explaining medical and social information to patients and families in clear terms

- LMSW (Licensed Master Social Worker)
