Immediate need for a talented Care Management Support Coordinator II . This is a 04+months contract opportunity with long-term potential and is located in U.S(Remote- EST). Please review the job description below and contact me ASAP if you are interested.

Pay Range: $23 - $24/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:
- Outreach members telephonically (about 30-40 calls a day).
- Answer incoming calls from members or Client associates.
- Create Case, Create Tasks, Create Referrals in member s chart in TruCare.
- Complete screenings in TruCare.
- Filter Excel Spreadsheet .
- Utilize Outlook Calendar for daily planning.
- Document in TruCare .
- Assist Member is downloading Wellframe App and Onboarding.
- Educating members on Digital Case Management and Wellframe.
- Handle 80-100 calls per week (outbound and inbound calls).
- Complete screenings on all new members .
- Document all member interactions and outreach in Trucare .
- Onboard 5-10 members/week to Wellframe.
Key Requirements and Technology Experience:

- HealthCare
- Call Center
- 1-2 years of experience .
- MS Word, Excel, Outlook .
- Prior Call Center experience.
- Prior experience in a managed health plan or medical facility.
- Professional Spoken Communication Skills/customer & admin service skills/ organized .
- Typing/problem solving skills/team collaboration.
- Medical/Healthcare Knowledge.
- High School Diploma/GED .
