Position: Registered Dietician Services

SPECIFICATIONS

1.Purpose and Scope: The WV Department of Shared Administration Purchasing Division is soliciting bids on behalf of the Department of Health Facilities, John Manchin Sr Health Care Center to establish a contract for Registered Dietician Services.

3.Qualifications: Vendor, or Vendor s staff if requirements are inherently limited to individuals rather than corporate entities, shall have the following minimum qualifications:

3.1.Licensed as a Dietician in the State of West Virginia

4.Mandatory Requirements:

4.1Mandatory Contract Services Requirements and Deliverables: Contract Services must meet or exceed the mandatory requirements listed below.

4.1.1.1Vendor must have a minimum of ten (10) years experience.

4.1.1.2Vendor must have experience writing approved plans of correction, implementation of plans of correction, care plans, writing menus for long-term care facility with a population age ranging from 18 to 100+ and writing diets for patients with chronic and terminal illnesses.

4.1.1.3Vendor must have knowledge of Federal and West Virginia regulations and experience with Federal and West Virginia survey processes.

4.1.1.4 Vendor must provide a Registered Dietician ready to assess residents within (5) days of start of contract.

4.1.1.5 Vendor must provide services during normal business hours of 8:00AM to 4:00 PM Monday through Friday.

4.1.1.6 Vendor will see residents in person weekly at John Manchin Sr Health Care Center.

4.1.1.7Vendor will complete documentation and recommendations electronically within 24 hours of evaluation.

4.1.1.8Vendor will work a maximum of 40 hours per month. Additional hours must be approved through Administration prior to completion.

4.1.1.9Vendor will bill John Machin Sr Health Care Center a per hour rate for all services.

4.1.1.10Vendor must have a valid WV Dietician s license.

4.1.1.11Vendor must complete a Purchasing Affidavit.

4.1.1.12Vendor must complete a HIPAA/BAA.SPECIFICATIONS
