JOB SUMMARY

We are seeking a detail-oriented and technically skilled Epic GardenPlot Analyst to join our team. In this role you will be responsible for the analysis, design, implementation, optimization and support of Epic GardenPlot for a small provider organization on a shared Epic instance. You will collaborate closely with clinical, operational and IT teams to ensure the application effectively supports organizational goals related to care coordination, community programs, regulatory reporting requirements and value-based care.

DUTIES AND RESPONSIBILITIES

Analyze and assess business processes, workflows and data needs to implement and optimize Epic GardenPlot modules.

Configure, build and maintain GardenPlot functionality, including care management programs, registries, screenings, assessments and community-based programs.

Collaborate with operational leaders and care teams to design solutions that meet end-user needs.

Perform system testing, validation and troubleshooting to ensure reliability, usability and compliance with regulatory guidelines.

Develop and maintain reports, dashboards and data extracts related to GardenPlot workflows, registries and program outcomes.

Provide end-user support, including training, documentation and ongoing technical assistance.

Participate in project planning, go-live activities and post-implementation support.

Ensure data integrity and privacy in accordance with HIPAA and organizational policies.

Stay up to date on Epic GardenPlot updates, new features and best practices.


Requirements

Epic GardenPlot Certification is required

Minimum of 2 years of experience supporting or implementing Epic applications, preferably GardenPlot.

Strong understanding of clinical workflows, community health programs and/or social determinants of health initiatives.

Proficiency in data analysis, reporting tools and registry management.

Strong problem-solving skills with attention to detail.

Excellent verbal and written communication skills, with the ability to effectively collaborate with cross-functional teams.

Ability to manage multiple priorities and work independently in a fast-paced environment.

Experience with other Epic modules (e.g., Health Planet, Care Management, SDOH tools) preferred.

Background in healthcare IT, community health or case management preferred.

Familiarity with value-based care initiatives and population health strategies preferred.

SQL, Crystal Reports or other reporting/analytics tools experience preferred.
