Care Management Specialist | 2+ Years of Experience | Remote/Fieldwork | Northside of Chicago | Contract to Hire

Walker Healthforce is seeking a Care Management Specialist with 2+ years to support a healthcare client remotely based out Northside of Chicago . This is a 6-month contract to hire opportunity.

START DATE: ASAP
HOURS/SCHEDULE: M-F; 8:00 AM - 5:00 PM CST 
WORKER TYPE: Full-Time W2

CORE REQUIREMENTS:

  • Must be local to the Northside of Chicago (Zip code - 60640) and willing to travel to members living in long-term care facilities
  • Current, valid, unrestricted Licensed Professional Counselor (LPC), Licensed Clinical Professional Counselor (LCPC), Licensed Master Social Worker (LMSW/LSW), or Licensed Clinical Social Worker (LCSW) with 2 years direct clinical care to the consumer in a clinical setting.
  • Plus 4 years wellness or managed care experience presenting clinical issues with members/physicians.
  • Experience handling medical management programs and health educations programs in an independent manner.
  • Knowledge of the health and wellness marketplace and employer trends.
  • Perform Transition of Care services and support members post-discharge
  • Analytical experience including medical data analysis.
  • PC proficiency to include Word, Excel, and PowerPoint, database experience and Web based applications.
  • Current state driver license, transportation, and applicable insurance
WE CONSIDER IT A BONUS IF:
  • 3 years clinical experience
  • Patient education experience
  • Condition Management experience

JOB RESPONSIBILITIES:

  • Completing in-person visits to members living in long-term care facilities
    • Conducting health assessments
    • Creating care plans
    • Educating members on health care programs to address their personal health plan needs
    • Engaging the member in discussion of adherence to personal health goals
