Job Details

Seeking a Community Health Improvement Plan (CHIP) Coordinator to support County Health Councils in developing, implementing, and monitoring Community Health Assessments (CHA) and Improvement Plans. This role will strengthen local efforts to address health priorities and promote health equity.
Key Responsibilities:

  • Support CHA/CHIP program development and implementation

  • Provide subject matter expertise and technical assistance to Health Councils

  • Coordinate with local, regional, and state partners

  • Lead outreach, relationship-building, and stakeholder engagement

  • Assist with new health initiatives and promote best practices

  • Attend council meetings and share success stories and updates

Requirements:

  • Experience in public health program development and coordination

  • Strong communication and relationship-building skills

  • Ability to support collaborative community health initiatives
