Description
* Conduct home health assessments and update systems accordingly.
* Engage with members to discuss and encourage adherence to personal health plans.
* Educate members on available health care programs and services.
* Support clinical teams with non-clinical functions to manage cases.
* Respond to inquiries from members and providers to address their needs.
* Maintain confidentiality and comply with privacy regulations.
* Communicate effectively with team members and management.
Key Responsibilities
* Perform outreach and follow-up with members on their health care plans.
* Build relationships to ensure compliance with care plans.
* Conduct member check-ins to review care plan goals.
* Process inquiries and update member information as required.
* Collaborate with internal teams to address member concerns.
* Notify technical support of any system issues encountered.
Qualifications
* Bachelor's degree in Social Work or Psychology, or equivalent healthcare qualifications.
* Experience in managed care systems or care coordination.
* Knowledge of medical terminology.
* Proficiency with general office software.
* Strong customer service and communication skills.
* Valid driver s license and willingness to travel.
