  • We are seeking a dedicated and skilled Physical Therapist for our Home Health division to provide high-quality, patient-centered care in the home setting.
  • This role is essential in fostering a positive rehabilitative environment for patients recovering from illness, injury, or surgeries. As a Physical Therapist in home health, you will evaluate each patient's physical condition, develop individualized treatment plans, and implement therapy interventions aimed at maximizing each patient’s independence and quality of life.
  • You will engage closely with patients, families, and caregivers to educate them about their conditions and the necessary steps for recovery.
  • This position demands effective communication skills, clinical expertise, and a compassionate approach as you work with individuals of varying needs and backgrounds. The successful candidate will possess a strong understanding of physical therapy practices and demonstrate the ability to adapt therapies to suit the home environment.

Your contribution will play a critical role in enhancing their mobility, functional capabilities, and overall well-being.
•    Evaluate patients through physical assessments and medical history
•    Develop personalized treatment plans based on individual needs and goals
•    Implement therapeutic exercises and training programs
•    Educate patients and their families on injury prevention and wellness
•    Monitor and document patients' progress toward rehabilitation goals
•    Collaborate with other healthcare providers to ensure holistic patient care
•    Maintain accurate patient records and documentation in compliance with regulations
 
Qualifications:
  • Master’s degree in physical therapy
  • 1 year of home health experience as a physical therapist
  • Currently licensed to practice as a physical therapist within the state of agency operation and compliant with accepted professional standards and practices
  • Must possess and maintain valid CPR certification while employed in a clinical role
  • Must be a licensed driver who can travel to all business locations
