CONTRACTOR RESPONSIBILITIES:

6.1. SERVICE TASKS: Contractor will perform the following tasks in the time and manner specified:

6.1.1 Design model objects (tables, views, indexes) daily as assigned by the Department.

6.1.2 Develop ad-hoc queries on MSSQL Databases daily as assigned by the Department.

6.1.3 Develop new DB objects in Transact-SQL (T-SQL) in support of programming daily as assigned by the Department. 6.1.4 Manage production promotion for application and SQL daily as assigned by the Department.

6.1.5 Add and Update Product Backlog items and complete tasks daily as assigned by the Department.

6.1.6 Process change tickets to implement system changes daily as assigned by the Department.

6.1.7 Provide change management in a code promotion role in partnership with development team daily as assigned by the Department.

6.1.8 Provide analytical services on systems to implement user-defined requirements daily as assigned by the Department.

6.1.9 Participate in backlog grooming, sprint planning, retrospective and review meetings as assigned by the Department.

6.1.10 Develop and document code per Division development standards daily as assigned by the Department.

6.1.11 In collaboration with Department's Division of Disease Control and Health Protection (DCHP) information technology teams; design, develop, and code based on business requirements for the DCHP Master Person Index (MPI) project as part of the overall strategic effort for integration and interoperability of its systems and software daily as requested by the Department.

6.1.12 Work with Division resources to standardize collection and storage of DCHP shared data daily as requested by the Department.

6.1.13 DATA SECURITY AND CONFIDENTIALITY TASK:

The Contractor, its employees, subcontractors, and agents must comply at all times with all Department data security procedures and policies in the performance of this scope of work as specified in the Data Security and Confidentiality document attached to the purchase order.

6.2. DELIVERABLES:

Contractor will complete and submit the following deliverables to the Department in the time and manner specified:

6.2.1. Monthly: Provision of Senior Web Application Developer Services in the time and manner specified in Tasks 6.1.1-6.1.12.

8. PERFORMANCE MEASURES AND FINANCIAL CONSEQUENCES:

All deliverables and related tasks must be completed 100% as specified. Failure to satisfactorily complete or submit a deliverable in the time and manner specified will result in a financial consequence as indicated below:

8.1. Failure to complete and submit Tasks 6.1.1.-6.1.12 in the time and manner specified will result in a payment reduction equal to 7 percent of the total monthly invoiced amount.

8.2. Failure to comply with Task 6.1.13, DATA SECURITY AND CONFIDENTIALITY will result in a payment reduction equal to 7 percent of the total monthly invoiced amount for each month the Contractor is not in compliance.

9. CONTRACTOR TRAVEL REIMBURSEMENT: Reimbursement for travel expenses is authorized only when approved in advance by the Department Contract Manager and conducted in accordance with Section 112.061, Florida Statutes and Department Internal Operating Procedure (IOP) 56-37.
