Outpatient Primary Care - FQHC

Specialty: Family Practice
Location: Ventura County California (60 miles outside of Los Angeles) Multiple locations.
# of Openings: 5
Provider Type: MD/DO FM and NP/PA
Dates: As soon as credentialed (takes 1-2 weeks) for 2 months to start with extensions if good fit. Prefer candidates who can start in October or mid November.
Certs required: BLS, DEA, CA License. Must be Truly BE or BC
Schedule: Mon-Fri 9a-5:30p 40 hours per week. 1 late night shift on Mon or Thurs from 11:30a-8p
Details: FQHC located in Ventura County with multiple locations. Every provider has 2 MA s assigned to them daily. Client does not want any providers with malpractice or background issues.
Patient Volume: 21 per day capped
EMR: NextGen
Expenses: All-Inclusive deal

Title: Outpatient Primary Care Provider

Position Overview: We are seeking a dedicated and compassionate Outpatient Primary Care Provider to join our team at [Name of the FQHC]. As an FQHC, we are committed to providing comprehensive, high-quality healthcare services to underserved populations in our community. The primary responsibility of the Outpatient Primary Care Provider will be to deliver primary medical care services to patients of all ages in an outpatient setting.

Key Responsibilities:

  1. Provide comprehensive primary medical care services to patients, including but not limited to: preventive care, diagnosis and treatment of acute and chronic illnesses, management of chronic conditions, and coordination of care with specialists as needed.
  2. Conduct thorough medical assessments, including physical examinations, medical history reviews, and diagnostic tests, to formulate accurate diagnoses and develop appropriate treatment plans.
  3. Collaborate with a multidisciplinary team of healthcare professionals, including nurses, nurse practitioners, physician assistants, social workers, and others, to ensure holistic and coordinated patient care.
  4. Educate patients and their families about their health conditions, treatment options, preventive measures, and lifestyle modifications to promote optimal health and well-being.
  5. Utilize electronic health record (EHR) systems to document patient encounters, maintain accurate and up-to-date medical records, and facilitate communication and information sharing among healthcare team members.
  6. Participate in quality improvement initiatives and adherence to evidence-based clinical guidelines to enhance the quality and effectiveness of patient care delivery.
  7. Stay abreast of current developments in the field of primary care medicine through continuing medical education (CME), professional development activities, and participation in relevant conferences and seminars.

Qualifications:

  • Medical degree (MD or DO) from an accredited medical school.
  • Board certification or eligibility in Family Medicine, Internal Medicine, or Pediatrics.
  • Valid state medical license and DEA registration.
  • Completion of residency training in an accredited primary care specialty program.
  • Experience working in a community health center or FQHC setting preferred but not required.
  • Strong interpersonal and communication skills, with the ability to interact effectively with patients from diverse cultural and socioeconomic backgrounds.
