Job Title: Care Manager - Health Plan
Location: Remote (CST hours)
Schedule: Monday Friday, 8:00 5:00 pm CST
Job Overview:
This position focuses on comprehensive care management, including assessment, care planning, and coordination of services for members with moderate to high behavioral health needs. The Care Manager will oversee the delivery of person-centered care plans, monitor outcomes, and collaborate with internal and external resources to ensure quality, cost-effective services.
Key Responsibilities:
Conduct health risk and comprehensive needs assessments
Develop and implement patient-centered care plans
Coordinate services across multiple providers and community resources
Provide telephone triage, crisis intervention, and prior authorizations
Educate members, families, and providers on care strategies
Oversee non-licensed care support teams
Continuously monitor and adjust care plans to close gaps and improve outcomes
Act as an advocate to address barriers to care
Participate in interdisciplinary team meetings and process improvement initiatives
Required Qualifications:
Active, valid license (e.g., LCSW, LMFT, LMSW, LPC, RN, or equivalent)
5+ years of experience in behavioral health care management or utilization management
Proficient with Microsoft Office and typing skills
Strong knowledge of clinical guidelines and behavioral health best practices
Preferred Skills:
Knowledge of DSM and ASAM
Experience working with Medicaid, Medicare, and Whole Health populations
