Role: Project Manager Clinical Programs Location: Martinez, CA (3 days onsite) Duration: 6 Months Contract to Hire
Summary: The Client is seeking a Project Manager for projects that support clinical programs including Quality Incentive Program (QIP), Provider Documentation, and Hierarchical Condition Category (HCC) coding initiatives. QIP is a statewide performance-based payment program that enables public healthcare systems to earn incentive payments by improving quality and demonstrating performance on key clinical, population health, and utilization measures. The Project Manager will drive greater efficiency, accountability, and performance across QIP operations. Focus areas include streamlining governance and program structure, improving performance management, and enhancing cross-functional coordination to optimize operational processes and use clinical expertise effectively. The Project Manager will collaborate with Quality, Clinical, Population Health, Business Intelligence, and Finance teams to ensure CCH maximizes QIP performance and funding outcomes.
Key Responsibilities:
- Lead project management and optimization of QIP operations across CCH.
- Design and implement an efficient QIP program structure and governance model.
- Align QIP roles, responsibilities, and workflows across functional teams.
- Monitor performance across all QIP metrics using dashboards and tracking tools.
- Facilitate regular QIP performance reviews with stakeholders and leadership.
- Identify and prioritize high-impact improvement opportunities tied to financial outcomes.
- Ensure timely and accurate submission of QIP deliverables.
- Promote a data-driven, proactive approach to QIP management and decision-making.
- Utilize organizational tools and process developed by Office of Strategic Initiatives
- Coordinate project meetings including setting meeting agenda, notes and action items
- Support additional clinical program initiatives as assigned
Qualifications:
- Bachelor's required; Master's in Nursing, Behavioral Science, Public Health, or related field preferred
- 4+ years PM/analyst experience in healthcare, including &Client;2 yrs performing quality measurement or quality improvement activities
- Understanding of QIP, Pay-for-Performance (P4P), QIP measures, and value-based care models.
- Experience managing cross-functional healthcare quality or performance improvement projects.
- Strong collaboration, communication, and organizational skills.
- Experience working with clinical leadership, BI/analytics teams, and financial stakeholders.
- Familiarity with Epic system(preferred).
- Certifications: PMP or Certified Scrum Master (preferred).
