About the Project

The State of Connecticut Department of Developmental Services (CTDDS) is seeking an experienced Project Manager to lead the Care Case Management workstream. This initiative will modernize, customize, and automate person-centered service planning for individuals with intellectual disabilities by implementing a system built on the Microsoft Power Platform in collaboration with CTDDS and a third-party vendor.

Key Responsibilities

  • Develop an understanding of CTDDS, its mission, and the populations it serves.
  • Collaborate with stakeholders including case managers, families, support providers, and other end users.
  • Analyze current systems, identify requirements, perform gap analysis, and manage end-user communications.
  • Lead and manage application and data migration projects in a Care Case Management environment.
  • Communicate effectively with business owners, IT teams, and leadership.
  • Facilitate meetings, testing, and test preparation.
  • Create and maintain project schedules, status reports, and meeting agendas.
  • Ensure HIPAA compliance in managing electronic health records (EHR), electronic medical records (EMR), and patient case management.

Required Qualifications

  • Strong technical project management experience in Care Case Management environments (social work, health & human services, or hospital settings).
  • Hands-on experience with HIPAA-constrained data elements, EHR, EMR, and patient case management.
  • Proven expertise in application and data migration projects.
  • Exceptional written and verbal communication skills.
  • Strong analytical, judgment, and decision-making abilities.
  • Proficiency in PowerPoint, Excel, and other analytical tools.
  • Ability to collaborate across functions, sectors, and geographies.
  • Strong time management to deliver high-quality work on schedule.
  • Experience with technology-driven change management initiatives.

Nice to Have

  • Experience with Monday.com for project management.
  • Familiarity with person-centered practices/strategies in healthcare or human services.

Additional Notes

  • Role can be fully remote Eastern Time Zone candidates strongly preferred.
  • Local CT candidates are a plus for occasional onsite meetings (travel costs not covered).
