Care Manager

Job Responsibilities

• Under the Supervision of the Manager of Care Management, the Care Manager is responsible for the overall accountability for the identification of members appropriate for care management services.
• Completes comprehensive assessments and gathers pertinent information about the members needs by interviewing the members, appropriate family members, physicians and other members of the member's Care Team to develop, monitor, and evaluate the member s Individualized Care Plan and update as needed.
• Presents members in ICT rounds providing updates on members current health status, questions, or concerns.
• Serves as an advocate for the member and acts as a liaison between the member and other community-based agencies, facilities, providers, and practitioners in coordinating the member s care.
• Supports care post-transition of care for members received from the transition of the care team.
• Monitors appropriate utilization of the member s benefits and coordinates services with other payer sources.
• Perform all other duties and projects assigned.

Skills

• Minimum of one year of care management experience

Education

• Bachelor s Degree Preferred.
• Associate's Degree Required.
• Licensed RN or Licensed Social Worker - Must be licensed in PA; either directly or through a compact license.
