Responsibilities include, but are not limited to:

·        Provide subject matter expertise on QPHE’s project to leverage NC HealthConnex for quality and population health, including project design, the implementation of healthcare provider supports, and developing strategies for future phases of work.

·        Lead related strategic efforts related to the collection and exchange of data to support care management programs and programs that address social drivers of health

·        Serve as liaison between QPHE’s Population Health staff, Healthy Opportunities Pilot staff, and the core staff leading the NC HealthConnex project

·        Ensure technology and business partners have formative information to plan and adapt their work in adherence to best practices, current policies, and stakeholder demands
