Description
- The Tennessee Department of Health, Division of Family Health and Wellness is hiring a Public Health Program Director 1.
- Public Health Program Director
- Job Type Full-time. This is a one-year position with the possibility of renewal. The role is primarily remote but requires in-person attendance at trainings, meetings, and other engagements at the supervisor's discretion (approximately once every two months). The position reports to the Family Health and Wellness Quality Improvement Director. The office is located at the Andrew Johnson Tower in Nashville, TN, and travel to the central office is non-reimbursable.

About the Role
- Tennessee is at the forefront of enhancing Health-Related Social Needs (HRSNs) screening and referral processes to ensure individuals and families can access high-quality, comprehensive, and coordinated services through a no-wrong-door approach. HRSNs encompass critical social and economic factors-including access to food, housing, transportation, utilities, and financial assistance-that directly influence health outcomes. This initiative, in partnership with TennCare, Tennessee's Medicaid agency, aims to implement a Closed-Loop Referral System (CLRS) using the FindHelp digital platform across three key programs:
- Community Health Access and Navigation in Tennessee (CHANT)
- Evidence-Based Home Visiting (EBHV)
- Viral Hepatitis
- The long-term goal is to establish a seamlessly coordinated system of care across government agencies and community-based organizations, ensuring that individuals and families in need are effectively connected to the right resources through a streamlined and integrated approach. These three programs serve as the first cohort of Tennessee Department of Health (TDH) programs leading this initiative, with the potential to enhance service delivery and coordination on a statewide scale.
- The Public Health Program Director 1 will serve as the day-to-day lead for this initiative, overseeing implementation, stakeholder engagement, and coordination across multiple agencies and organizations.
Responsibilities
- Lead project implementation across CHANT, EBHV, and Viral Hepatitis, ensuring alignment with program goals.
- Act as the primary liaison with the FindHelp team to facilitate collaboration and resolve implementation challenges.
- Coordinate with TennCare, IT teams, and data governance staff to support system integration and operational efficiency.
- Develop and implement training programs for staff, partners, and community-based organizations to maximize effective use of the referral system.
- Work closely with the community engagement coordinator to develop and execute an engagement strategy that supports platform adoption.
- Monitor referral data and platform analytics to assess usage, identify challenges, and support continuous improvement.
- Organize and participate in regular meetings to track progress, refine strategy, and address support needs.
- Identify and address barriers to adoption, particularly in areas with limited resources.
- Support efforts to ensure seamless integration of the referral system into existing workflows and processes.
- Foster collaboration across state and local agencies to strengthen referral networks and service coordination.
- Provide support in other Division of Family Health and Wellness (FHW) initiatives as needed, ensuring alignment with broader program goals and emerging priorities.
- Oversee workflow analysis, IT configurations, and potential system integrations as needed.
Qualifications
- Graduation from an accredited college or university with a bachelor's degree and at least three years of experience in public health, health program implementation, or project management.
- A master's degree in public health (MPH) or a related field is strongly preferred.
- Proven ability to manage multi-stakeholder projects, particularly within health or human services.
- Strong coordination and organizational skills to oversee complex projects involving multiple partners.
- Exceptional attention to detail to ensure accurate tracking of workflows and project milestones.
- Ability to engage and support teams, fostering collaboration and sustained participation.
- Experience in program coordination, community engagement, or care coordination.
- Familiarity with data analysis and reporting to inform program monitoring and decision-making.
- Proficiency in Microsoft Office programs (Word, Excel, Outlook, Teams) and comfort using project management platforms such as Asana.
- Experience working with digital referral platforms, electronic health records, or case management systems is a plus.
