RN Care Manager Full-Time | Hybrid Role
Location Requirement: Must reside within 15 20 miles of any of the following areas in Massachusetts:
Boston, East Boston, Chelsea, Revere, Watertown, Roslindale, Roxbury, Dorchester, Hyde Park, Everett

Job Overview:

Join a purpose-driven care management team supporting family caregivers and their loved ones. This full-time hybrid role combines in-person home visits with remote coaching/documentation. You will not be responsible for direct patient care but will focus on caregiver education, support, and clinical coaching.

Compensation & Perks:

  • Base Salary: $90,000 $105,000 (based on experience)
  • Sign-on Bonus: $5,000
  • Mileage Reimbursement: Yes
  • Work Schedule: Monday Friday only (No nights, weekends, or on-call)
  • Benefits: Full benefits provided by the client
  • Work Type: Hybrid field visits + remote documentation
  • Tools Provided: Tablet and software access for documentation
  • Training: 4-week orientation + shadowing provided at start

Key Responsibilities:

  • Conduct home visits every other week to approx. 45 caregivers (avg. 16 20 visits/week).
  • Perform safety checks, monitor changes in health, and provide clinical coaching.
  • Review medications, offer resource referrals, and guide on best practices.
  • Use provided tablet/app to document caregiver interactions and progress.
  • Support caregivers in reducing burnout and improving care outcomes.
  • Maintain compliance through timely and accurate documentation.
  • Work independently and manage your own schedule within your assigned area.

Mandatory Requirements:

  • Active Registered Nurse (RN) License in Massachusetts
  • Valid Driver's License and Auto Insurance
  • Minimum 2 years of direct care experience (elder care, disabilities, LTC, hospice, or home health)
  • Strong documentation, time management, and communication skills
  • Must be tech-savvy able to use apps and web-based platforms for notes and tracking

Preferred Qualifications:

  • Case management, care planning, or assessment experience
  • Bilingual Spanish (Preferred but not mandatory. Bilinguals accepted for any location)

Additional Notes:

  • Direct patient care is NOT involved the focus is on supporting family caregivers.
  • Candidates must be comfortable working independently and managing a personal schedule.
  • Interviews within 24 48 hours of name clearance.
  • Must be open to full-time only no part-time options available.
