Responsibilities

• Perform therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs to promote recovery.
• Manage discharge planning to ensure seamless transition of care from hospital to home or community settings.
• Document all patient interactions accurately using electronic health records (EHR) systems while adhering to clinical documentation standards.
• Collaborate closely with physicians, nurses, occupational therapists, and other healthcare professionals to optimize patient outcomes.
• Educate patients and their families on treatment techniques, injury prevention strategies, and home care routines tailored for geriatric, pediatric, neurological, orthopedic, or outpatient rehabilitation needs.

Requirements

• Valid license or certification to practice as a Physical Therapist in the relevant jurisdiction.
• Proven experience working with diverse patient populations including geriatrics, pediatrics, neurology, orthopedics, and outpatient rehabilitation.
• Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques.
• Experience with inpatient hospital settings, outpatient clinics, home health services, or outpatient rehabilitation centers is highly desirable.
• Proficiency in electronic health records (EHR) and electronic medical records (EMR) systems for accurate documentation and treatment tracking.
• Ability to develop effective treatment plans based on clinical assessments and patient needs.
• Excellent client communication skills to foster trust and motivate patients throughout their recovery process.
• Familiarity with discharge planning processes and working within clinical documentation standards is essential.
