Pay Range: $25 - $27/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:
- Provide resource support to members, connecting them to local services such as employment, housing, independent living, justice, and foster care.
- Conduct assessments and develop care plans based on individual needs and industry best practices.
- Document outreach notes and care plans in an electronic health record system.
- Collaborate with internal and external stakeholders, including community-based care agencies and healthcare management teams.
- Participate in training and onboarding, ensuring compliance with care management policies and documentation standards.
Key Requirements and Technology Experience:
- 2 – 4 years of related experience in child welfare or a similar field.
- Bachelor’s Degree in a relevant discipline.
- Experience connecting families and children to local resources for various services.
- Proficiency in Microsoft Office Suite (Outlook, Word, Excel).
- Experience in health plans, foster care, or adoption services.
