Pay Range: $25/hr - $27/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:
- Shift: 8am - 5pm, Monday through Friday; Weekend work may be needed during emergency situations.
- Provide care management to assist with coordination of care, including assessing health care needs and referring to health plan providers.
- Educate members on benefits and expanded benefits while offering community resources to address social determinants of health needs.
- Manage a caseload of 100 members, documenting in an electronic health record, completing outreach notes, assessments, and care plans.
- Ensure compliance with care management policies/procedures, including outreach and documentation expectations, with monthly audits to verify chart compliance.
- Participate in a comprehensive onboarding process, including training on tools and processes over a 4-week period.
Key Requirements and Technology Experience:
- 2+ years of experience in child welfare.
- Experience connecting families/children to local resources for various services, including employment, housing, independent living, justice, and foster care.
- Proficiency in Microsoft Office Suite (Outlook, Word, Excel).
- Nice to have: Health plan experience; Foster care and/or adoption experience.
