Job Title: Head of Chronic Care Management (CCM) Program

Role Overview
The Head of Chronic Care Management (CCM) is responsible for the overall strategy, execution, and performance of Kencor Health’s Chronic Care Management program. Reporting to the COO, this role provides end-to-end leadership across care delivery, clinical quality, regulatory compliance, financial performance, people management, and program growth enablement. The role works in close collaboration with the Medical Director to ensure clinical integrity, adherence to standards of care, and continuous improvement of care models. In addition, the Head of CCM supports sales and marketing efforts by serving as a subject-matter expert, contributing to program positioning, and ensuring the CCM program is compliant, scalable, and accurately represented to provider partners.
Key Responsibilities
Program Leadership & Governance
Own the objective, operating model, and performance of the CCM program.
Translate organizational objectives into scalable CCM strategies and execution plans.
Establish program governance, KPIs, and a regular operating cadence.
Serve as the accountable CCM program leader reporting to the COO.
Clinical Partnership & Care Model Oversight
Partner closely with the Medical Director to define and maintain CCM care models, clinical pathways, and escalation protocols.
Ensure care delivery aligns with evidence-based standards and regulatory requirements.
Incorporate clinical guidance into workflows, training, and quality improvement initiatives.
Balance clinical quality, operational efficiency, and financial performance.
Collaborate closely with Providers Practices (physicians and practice managers) to align
CCM workflows with practice operations, care plans, and patient needs.
Care Management, Care Coordination & Case Management
Oversee delivery of care management, care coordination, and case management services across the CCM program.
Ensure effective patient engagement, individualized care planning, medication reconciliation, appointment coordination, and follow-up.
Drive consistency and quality in patient outreach, documentation, and care transitions.
Monitor patient outcomes and intervene when gaps in care or risk escalation are identified.
Leadership & Team Oversight
Provide leadership and oversight for CCM Care Managers, Registered Nurses (RNs), Medical Assistants (MAs), and other CCM staff.
Define staffing models, productivity standards, capacity planning, and role clarity.
Lead hiring, onboarding, training, performance management, and professional development.
Foster a culture of accountability, collaboration, and patient-centered care.
Regulatory Compliance & Risk Management
Serve as the accountable owner for CCM regulatory compliance.
Ensure adherence to CMS and payer requirements related to patient consent, documentation, time tracking, billing, and supervision.
Monitor regulatory changes and translate them into operational workflows, training, and controls.
Maintain audit readiness and partner with Compliance and Legal teams to mitigate program risk.
Financial Performance & Profitability
Own CCM program financial performance, including revenue, cost structure, and margin targets.
Monitor enrollment, engagement, time capture, and reimbursement effectiveness.
Partner with Finance to optimize staffing ratios, pricing structures, and capacity utilization.
Identify opportunities to improve efficiency and profitability while maintaining care quality and compliance.
Sales & Marketing Support
Act as the CCM subject-matter expert supporting Sales and Marketing efforts.
Contribute to program messaging, collateral, and provider education materials.
Participate in sales calls, provider meetings, and implementation planning as needed.
Ensure program capabilities, workflows, and compliance posture are accurately represented externally.
Data, Reporting & Continuous Improvement
Define, track, and report KPIs across clinical outcomes, compliance, operations, staffing, and financial performance.
Use data to identify trends, gaps, and improvement opportunities.
Partner with Technology and Analytics teams to enhance CCM workflows, automation, and reporting.
Drive continuous improvement across the CCM program.
Qualifications
Required
Active Registered Nurse (RN) or Nurse Practitioner (NP) license with formal clinical education and training.
Minimum of 5 years of leadership experience in chronic care management, care coordination, case management, or population health programs.
Proven experience in care management, care coordination, and case management within chronic or longitudinal care settings.
Demonstrated success leading CCM or care management operations with program-level accountability.
Strong working knowledge of CCM regulations, documentation standards, and reimbursement mechanics
Experience leading multidisciplinary care teams, including Care Managers, RNs, MAs, and related staff.
Ability to design and optimize care workflows balancing clinical quality, compliance, patient experience, and operational efficiency.
Experience managing regulated healthcare programs with accountability for clinical outcomes and financial performance.
Strong cross-functional communication, collaboration, and leadership skills.
Preferred
Experience scaling CCM programs across multiple provider partners.
Familiarity with Remote Patient Monitoring (RPM) and integrated longitudinal care models.
Experience supporting sales and marketing efforts in a healthcare services or health-technology environment.
Experience collaborating closely with Medical Directors or clinical leadership.
