Job Summary (List Format): Senior Social Worker – AZDVS, Flagstaff, AZ

General Overview:
- Serve as a compassionate Senior Social Worker at the Arizona Department of Veterans' Services (AZDVS).
- Support, serve, and honor veterans, service members, and their families.
- Provide high-quality assistance to veterans for improved quality of life.

Key Responsibilities:
- Plan, organize, and supervise the social services program at Arizona State Veteran Home.
- Ensure timely completion of assessments and documentation per federal/state regulations and facility policies.
- Oversee compliance with PASRR (Preadmission Screening and Resident Review) processes.
- Supervise, assign, and review the work of Social Services staff.
- Conduct staff interviews, selection, and performance evaluations (monthly coaching, annual appraisals).
- Address staff issues and recommend disciplinary actions as needed.
- Lead and oversee clinically appropriate discharge planning.
- Monitor and ensure timely delivery of Medicare notices to beneficiaries.
- Complete consults and referrals to VA specialists.
- Collaborate with residents, families, and staff to ensure adherence to Resident’s Rights.
- Assist with department budget projections and monitor expenditures.
- Maintain a personal caseload of assigned veterans/members.
- Attend meetings and participate on committees.

Knowledge, Skills, and Abilities (KSAs):
- Knowledge of CMS and State regulations, social services programs, geriatric psychiatric and disease processes, psychotropic medications, restraints, community resources, governmental programs, long-term care billing, and MDS system.
- Skills in verbal/written communication, counseling, case management, resident assessment, care planning, analytical thinking, problem-solving, supervision, and management.
- Ability to establish/maintain professional relationships, use facility IT programs, maintain confidentiality, and document assessments and care plans.

Requirements:
- Master’s degree in social work (REQUIRED)
- Minimum one year of supervisory experience in a healthcare setting
- Experience in long-term care
- Active social work license from the Arizona Board of Behavioral Health Examiners
- Ability to obtain/retain valid AZ DPS Fingerprint Clearance Card
- Current negative TB skin test (within last 6 months or 1 year)
- Background check (HireRight) and facial biomatrix verification

Additional Preferences:
- Experience with long-term care and veteran populations
- Familiarity with Arizona Management System (AMS) and continuous improvement initiatives

Contract Details:
- 12-month contract, onsite, local to Flagstaff, AZ
- $33/hr, W2, all-inclusive rate
