About the Role
We are seeking a motivated and relationship-driven Outreach Specialist to support the successful enrollment of prospective participants into the PACE (Program of All-Inclusive Care for the Elderly) program.
The Outreach Specialist will be responsible for developing strong relationships within the local community, generating high-quality referrals, and supporting prospective participants throughout the outreach and enrollment process. This is a business-to-consumer sales and community outreach role requiring excellent communication, relationship-building, and sales skills.
Spanish-speaking ability is required.
Key Responsibilities
- Plan, coordinate, and execute community outreach, public relations, and engagement activities to achieve monthly referral, home visit, and enrollment goals.
- Build and maintain strong relationships with community organizations, community leaders, physicians, hospital discharge planners, post-acute facilities, and other referral sources.
- Identify and develop new community partnership and referral opportunities.
- Educate prospective participants, families, and community referral sources about PACE services and available programs.
- Collaborate with leadership to develop and execute an annual marketing and outreach plan aligned with enrollment goals.
- Generate and manage quality referrals that lead to participant enrollment.
- Demonstrate a strong understanding of the organization's outreach, marketing, and enrollment processes and playbook.
- Ensure compliance with all applicable healthcare marketing rules, regulations, and allowable marketing practices.
- Identify opportunities to improve outreach processes, referral generation, and enrollment outcomes.
- Maintain accurate and timely documentation and reporting using Salesforce, Athena, and Microsoft Office Suite.
- Collaborate closely with the Marketing, Outreach, and Enrollment team to support outreach and enrollment activities as needed.
- Protect participant privacy and maintain strict confidentiality of participant, family, and organizational information.
- Participate in required training, quality improvement initiatives, staff meetings, and occupational health requirements.
- Communicate regularly with leadership and team members regarding workload, priorities, and outreach activities.
- Maintain flexibility to work varied schedules, including occasional evenings, weekends, and overtime based on business needs.
Required Qualifications
- 2–5 years of relevant experience in community outreach, healthcare sales, business development, field sales, enrollment, or a similar customer-facing role.
- Fluent Spanish-speaking ability is required.
- Business-to-Consumer (B2C) sales experience is required.
- Strong relationship-building, communication, and interpersonal skills.
- Experience developing and maintaining relationships with community organizations and referral sources.
- Ability to meet referral, outreach, and enrollment targets.
- Strong organizational skills with the ability to manage multiple priorities.
- Experience using CRM and productivity tools such as Salesforce, Athena, and Microsoft Office Suite.
- Ability to work independently while collaborating effectively with cross-functional teams.
- High School Diploma or equivalent required.
- Ability and willingness to work a flexible schedule, including evenings and weekends when needed.
Preferred Qualifications
- Previous experience in healthcare, senior care, community health, PACE programs, Medicare, Medicaid, or participant enrollment.
- Experience working with physicians, hospitals, discharge planners, post-acute care facilities, or community-based organizations.
- Experience in healthcare marketing, community engagement, or referral development.
- Strong understanding of the local senior care or healthcare services market.
