Job DetailsFull time Role in Case Management
Work Schedule Monday through Friday from 830 am to 5 pm with rotating weekend coverage
RNCM collaboratively implements and facilitates patient care plans from pre-hospital to post-discharge while managing an assigned unit case load.
RN Case Manager will be responsible for the assessment, planning and monitoring options and services to effectively provide an individualized plan for the patient across the continuum of care.
Using independent judgment and discretion, the continuum must be reassessed as an ongoing process for the individual being served by the RN Care Manager and Social Worker Care Manager.
RNCM Utilizes clinical knowledge and competence, positive communication skills (both verbal and written), problem solving and conflict resolution techniques, ability to effect change, strong skills in assessment, organization, and time management.
RNCM will enforce the proper application of hospital policies and procedures regarding appropriateness and timeliness of clinical care given.
QUALIFICATIONS
Minimum 3-5 years relevant clinical medical surgical, ICU or ER experience preferred.
Experience in managed care performing utilization review and discharge planning preferred.
CCM certification preferred.
Current NYS RN license required.
