We are seeking an experienced Care Manager – ED to coordinate patient care across the continuum of services, ensure appropriate utilization of healthcare resources, facilitate discharge planning, and support quality patient outcomes. The ideal candidate will collaborate with interdisciplinary teams to assess patient needs, coordinate services, and ensure compliance with organizational and regulatory standards.
Key Responsibilities:
Coordinate patient care and services across the continuum of care.
Perform utilization review to ensure appropriate use of healthcare resources.
Develop and implement discharge plans for safe patient transitions.
Collaborate with physicians, nurses, social workers, and other healthcare professionals.
Assess patient needs and identify barriers to care.
Facilitate communication among patients, families, and care teams.
Maintain accurate clinical documentation and patient records.
Ensure compliance with organizational policies and regulatory requirements.
Participate in quality improvement initiatives.
Required Qualifications:
Bachelor''s Degree in Nursing (BSN) from an accredited program.
Current Registered Nurse (RN) license in the State of New York.
Minimum of 5 years of clinical experience as a Registered Nurse.
Experience in Care Coordination, Utilization Review, and Discharge Planning.
Strong computer literacy skills.
PRI and Screen Certification (required at the time of submission).
Primary Source Verification of RN License and Registration from NYSED.
Basic Life Support (BLS) certification.
