hope you’re doing well! I wanted to share an opportunity for a Registered Nurse (RN) – Home Health role with a leading healthcare organization in San Antonio, Texas.

Location: San Antonio, TX
Facility: Consolidated Home Health & Hospice

Schedule:

• Monday – Friday, 8:00 AM – 5:00 PM
• On-call: Once per month

Coverage Area:

• San Antonio and surrounding areas

🩺 Key Responsibilities:

• Perform patient assessments and develop individualized care plans
• Provide ongoing nursing care for acute and chronic conditions
• Coordinate care with physicians, therapists, and interdisciplinary teams
• Monitor patient progress and adjust care plans as needed
• Administer medications and treatments as prescribed
• Ensure accurate and timely clinical documentation
• Educate patients and families on disease management and care plans
• Supervise LPNs and support staff as required
• Ensure compliance with safety, infection control, and regulatory standards

Required Qualifications:

• Active RN License (Texas or Compact)
• Strong clinical assessment and patient care skills
• Ability to work independently in a home health setting

Preferred Skills:

• OASIS and Home Care Home Base (HCHB) experience
• Wound care and disease management
• Experience with:
  • IV antibiotics & PICC lines
  • Ostomy and Foley care
  • Wound vacs
  • Drains (JP drain, PleurX)
