Job Description

Our client is seeking a Engineering Manager to join their team. The platform works, and it carries the weight of accumulated decisions. The work is a mix of reimplementation and genuine greenfield development. Some of it means pulling that logic out of the database into tested services on Postgres. Some of it means building new systems from scratch as separate portals consolidate into one modular platform with agentic AI built in from the start.

This role runs that work day to day and still writes a real share of the code. You'll lead our US full stack engineers directly and serve as the US point of contact for our wholly owned engineering subsidiary in India. Expect to spend around 40% of your time hands-on. The rest goes to planning and delivery with the Director of Technical Product, and to holding both teams to the same engineering bar. You report to the CTO, who is technical and reads code and specs directly. The Director of Technical Product defines what we build and why. You're accountable for how it gets built and whether it ships well.

What you'll own:

  • Production code across the stack in TypeScript and Python, with a bias toward the hardest and least understood parts of the system
  • Technical planning for roadmap initiatives with the Director of Technical Product, then execution across the US and India teams, including the final call in design reviews when the team is stuck between approaches
  • Stored-procedure migration: reimplementing the business rule inside each procedure as tested application code, then cutting over without breaking the operational reporting that finance and our health plan clients depend on
  • Technical leadership on both greenfield builds and rebuilds, including the new Next.js member self-scheduling portal and the NestJS services behind clinician scheduling and dispatch
  • Engineering standards for code review, branching, testing and definition of done, applied the same way in the US and India
  • CI/CD that runs automated tests and security scanning on every change before merge, so deploys become routine
  • HIPAA-aware engineering as the default: least-privilege access, audit logging on PHI access, secrets in Secrets Manager, BAA-covered services only, and offshore PHI access controls enforced in code and at the database layer
  • Delivery metrics (deployment frequency, lead time, change failure rate, time to restore) and production quality for your team's services, including on-call and blameless postmortems
  • Management and mentoring of the US full stack engineers, including senior and lead developers, plus hiring for US and India roles against a technical bar you define
  • The working relationship with our India-based subsidiary: one shared backlog, joint sprint planning and reviews, a daily overlap window in your early morning, and written handoffs so work doesn't stall overnight
  • Architecture decision records and runbooks, kept current so system knowledge doesn't live in one person's head

First-year priorities:

1. Ship code to production in your first two weeks, and deliver a written assessment of engineering practices across both teams in your first 90 days, with a ranked list of what changes first

2. Put written standards for code review and definition of done in place across both teams, with baseline delivery metrics reviewed weekly with the CTO and Director of Technical Product

3. Get handoffs between the US and India to the point where the time difference no longer costs a day

4. Move the highest-revenue stored-procedure logic into application code, verified against existing reporting, with a staffed plan for the rest

5. Improve lead time and change failure rate measurably against your 90-day baseline, and get a growth plan in place for every engineer on both teams

Required qualifications:

  • 10+ years shipping production software you were accountable for after launch, with at least 3 years managing engineers while staying hands-on
  • Strong TypeScript across the Node ecosystem: Next.js on the front end and NestJS or a comparable server framework on the back. You still write production code and can hold your own in review with a senior engineer.
  • Genuine SQL depth in both Postgres and MySQL. You can read a query plan, tell when an ORM emitted something terrible, design a schema that holds up under reporting load, and plan a zero-downtime migration.
  • Python for services and data work, with type hints and tests
  • AWS at a level you can defend in detail: RDS, S3, IAM, Secrets Manager, containerized deploys on ECS or Fargate, and infrastructure as code in Terraform or CDK
  • A track record of raising a team's engineering standards, with evidence such as test coverage that went up or a deploy cadence that went from monthly to daily
  • Experience leading or partnering closely with an offshore engineering team across a large time difference, and making that arrangement work rather than tolerating it
  • Experience building software that handles PHI or similarly regulated data, with working knowledge of HIPAA technical safeguards

Preferred qualifications:

  • Healthcare experience, particularly Medicare Advantage risk adjustment or quality programs (HEDIS, Star ratings)
  • LLM work in production rather than in a notebook, including evaluation and cost control. Bedrock or Anthropic API experience is a plus.
  • A data migration or systems consolidation you led off a stored-procedure-heavy or otherwise legacy codebase
  • HL7/FHIR or X12 834/837 exposure
  • Telephony integrations, or consent capture under TCPA and 10DLC
  • Startup experience at a stage where you had to decide what not to build
  • HITRUST or SOC 2 audit experience from the engineering side

Salary/Rate: $60-$75/HR (depends on experience level). This is a contract to hire position with candidates expected to work 40 hours/ week. Sponsorship is not available for this role.
