• M-F: 8am - 5pm schedule

• 100% Outpatient Medicine

• Sick Visits, Medication Management, Chronic Disease Management, etc.

• Group has 5 locations and 21 other Family Practice providers

• Ohio State Medical License

• Must be Board Certified / Board Eligible Family Medicine
