Care Coordinator - Dual CCS/CLTS

Full-Time | Milwaukee County | Approximately 40 hours/week

Pay Range: $45,299 - $67,948

We are looking for a Care Coordinator - Dual CCS/CLTS to join our team and provide individualized care coordination to children, adolescents, youth, and adults with mental health, substance use, developmental, and/or co-occurring needs.

What You\'ll Do

As a Care Coordinator - Dual CCS/CLTS, you will:

  • Build trusting, respectful relationships with individuals and families and meet people where they are.
  • Provide trauma-informed, person-centered, and strengths-based care coordination.
  • Partner with individuals, families, natural supports, and community providers to develop and implement individualized recovery plans.
  • Connect individuals to mental health, substance use, healthcare, social, and community resources.
  • Complete referrals and coordinate services with therapists, healthcare professionals, case managers, and other providers.
  • Monitor progress toward each individual\'s self-identified goals and provide supportive coaching, psychosocial rehabilitation, and symptom-management support.
  • Conduct regular in-person visits, phone calls, and follow-ups.
  • Advocate for timely and appropriate services while empowering individuals to build on their strengths.
  • Help individuals access transportation and other resources needed to participate in services and appointments.
  • Respond calmly and compassionately to crises and challenging situations.
  • Maintain accurate and timely electronic documentation in accordance with agency, county, and regulatory requirements.
  • Complete required agency and county training.


What We\'re Looking For

We\'re looking for someone who is compassionate, dependable, organized, and passionate about helping people succeed.

Required:

  • Bachelor\'s degree in Social Work or a related field.
  • Valid driver\'s license, reliable automobile, and insurance meeting agency requirements.
  • Ability to pass required background, caregiver, driver\'s record, and drug screening.
  • Strong communication, organization, problem-solving, and time-management skills.

Preferred:

  • At least 1 year of experience working with individuals experiencing mental health, substance use, or co-occurring needs.
  • Master\'s degree in Social Work or a related field.
  • Bilingual/multilingual skills.
  • Experience working with Milwaukee County Behavioral Health Services.
  • Experience with care coordination, case management, behavioral health, or community-based services.

Work Environment & Schedule

This is a community-based position. Your day may include office work, home visits, community meetings, provider appointments, and connecting with individuals throughout Milwaukee County.

  • Approximately 40 hours per week
  • Flexible schedule based on the needs of individuals and families
  • Community-based work and travel throughout Milwaukee County
  • Transportation of individuals may be required
  • Ability to navigate stairs and work in a variety of community settings
