Job Details- Working on-site at a major medical center in Bronx NY, the Hospice Care Planner is responsible for evaluating and establishing a plan of care for patients referred for Hospice Care services.
- Provide education to patients, family members, and referral sources regarding the Hospice benefit and range of services.
- Collaborate with referral sources from a variety of Health care practice areas to coordinate Home and in-patient hospice services.
Qualifications:
- Licensed with current Registration to practice as Registered Professional Nurse in NYS is required.
- Bachelor's Degree preferred.
- Must have a minimum of two years experience in Hospice nursing with proficiency in physical assessment and interviewing skills.
