Summary:

The Case Manager assumes accountability and responsibility for managing the hospitalization of a caseload of patients from pre-admission through one week post-discharge.

Responsibilities:

  1. Assumes accountability and responsibility for managing the hospitalization of a caseload of patients from pre-admission through one week post-discharge.
  2. Facilitates coordination of care by all members of the health care team to facilitate achievement of appropriate outcomes.
  3. Completes educational responsibilities.
  4. Performs leadership duties.
  5. Performs quality assurance and performance improvement duties.
  6. Performs communication and networking duties.
  7. Performs systems based practice duties.
  8. Demonstrates the knowledge and skills necessary to provide care for the physical, psych/social, educational, and safety needs of the patients served regardless of age.
  9. Other duties as required.

Other information:

Technical Expertise

  1. Experience in case management is preferred.
  2. Experience working with all levels within an organization is required.
  3. Proficiency in MS Office [Outlook, Excel, Word] or similar software is required. Epic software or similar EMR software is required.

Education and Experience

  1. Education: Bachelor\'s degree in Nursing [BSN] is required; Master\'s degree is preferred.
  2. Certification: Registered Nurse licensure is required.
  • Current Health Care Provider BLS training from the American Heart Association is required.
  • CCM or CCCTM Certification is preferred.
  1. Years of relevant experience: 3 to 5 years is required.
  2. Years of experience supervising: None
