Summary

Description
Community-based care managers work with physicians, nurses, social workers, pharmacists and other caregivers to develop care plans that connect people to the right local resources and really make a difference. Organization provides independent physicians and practice managers with layers of support that saves time and delivers resources needed to improve care and help keep a practice operating smoothly.
This role is a hands-on, builder-focused position responsible for designing, building, and scaling internal FHIR-based clinical data acquisition capabilities. The organization is intentionally shifting away from heavy vendor dependence and looking to develop sustainable, in-house clinical data pipelines.
This is currently a leader and one-person execution role. While there is strong potential to build a team underneath this person in the future, the immediate need is for someone who can personally design, build, and scale clinical data integrations.
Leaders in this organization are doers, and this role is expected to be highly hands-on.
Key Responsibilities
Design and build internal FHIR-based clinical data acquisition pipelines
Work directly with the CTO to expand and scale FHIR capabilities beyond existing proof-of-concepts
Ingest and normalize clinical data from EMRs, including a broad range of systems (Epic, Athena, Allscripts, and smaller EMRs)
Reduce reliance on third-party vendors by developing internal capabilities for clinical data acquisition
Architect scalable solutions that can support multiple EMR integrations without being single-threaded to one vendor
Build and maintain data pipelines using Python and SQL to support healthcare and clinical data use cases
Apply high-level architectural thinking while remaining hands-on with implementation
Collaborate closely with internal stakeholders across technology, clinical data, product, project management, and quality teams
Communicate effectively with both technical and non-technical audiences and adapt messaging based on the audience
Operate independently, prioritize work effectively, and demonstrate strong judgment, emotional intelligence, and resource management
Required Experience & Skills
Hands-on experience building and scaling FHIR environments (not theoretical or academic exposure)
Direct experience working with clinical data sourced from EMRs
Strong Python and SQL skills applied to healthcare data
Experience designing or implementing clinical data pipelines in a payer or payer-adjacent environment (preferred)
High-level data architecture skills, with the ability to design scalable solutions
Experience working in cloud environments
AWS preferred
Google Cloud Platform or Azure acceptable
Ability to work independently in an environment with limited existing bandwidth and infrastructure
Strong communication skills and the ability to "read the room" with both technical and business stakeholders
Preferred Experience
Healthcare domain experience required
Payer experience strongly preferred, though not mandatory
Prior experience transitioning away from vendor-dependent data solutions is a plus
Comfortable operating without a formal team initially, with openness to people leadership in the future
What This Role Is Not
Not a pure people-management role (at least initially)
Not a purely strategic or advisory position
Not a generic data or BI role without deep healthcare and FHIR experience

Essential Functions
Coordinate with internal stakeholders and the value-based care vendor to ensure platform data meets business needs and quality standards. Lead the design, development, and implementation of analyses, insights, and products to support these requirements.
Direct ongoing development of enterprise care management and payer-based analytics, including supplemental reporting to payers, data integrity oversight, dashboard design, customer engagement, and project management. Manage multiple phases of product design and rollout, and support provider representatives in effectively using analytics to improve care coordination.
Partner with executive leadership and the value-based vendor to shape and implement the evolving product vision.
Develop new and standardized clinical interfaces for ingesting EMR data using standards such as FHIR and HL7.
Oversee team deliverables, ensure accuracy and prioritization of requests, and coordinate reporting to executives, business leads, and end users.
Lead reviews of data integrity and coding issues, assessing impact on product quality and designing or delegating corrective actions.
Collaborate across analytic teams on overlapping projects and responsibilities.
Lead and mentor team members, fostering continuous development, a positive work environment, and a culture of professional excellence.
Staff Management
Provide effective leadership in recruitment, selection, retention, training, evaluation, compensation, and professional development.
Ensure staff remain well trained and compliant with organizational and departmental policies, procedures, and goals.
