Role Overview

We are seeking a dedicated and organized Youth Transition Specialist to provide critical oversight for reentry practices within a juvenile correctional facility. In this role, you will serve as the primary bridge between incarceration and community success, ensuring every student has a clear, actionable roadmap for their future.

You will lead the implementation of life skills curricula, manage educational learning plans, and coordinate with state and private agencies to ensure a seamless transition for youth returning to their communities.

Key Responsibilities

• Curriculum & Guidance: Facilitate "Next Steps" life skills programming and evaluate student "Transition Consultations" guided by teaching staff.
• Reentry Planning: Participate in weekly Administrative Release Committees (ARC) to finalize and reinforce student reentry strategies.
• Case Management: Assist students in obtaining vital documentation, including Social Security cards, state IDs, birth certificates, and driver's licenses.
• Resource Connection: Partner with the Department of Workforce Development, HIRE, and mentoring programs to secure employment and apprenticeship opportunities.
• Community Advocacy: Represent the organization at conferences and meetings to build partnerships that leverage resources for transitioning youth.
• Long-term Support: Conduct follow-up outreach with released youth at 1, 3, 6, 9, and 12-month intervals to track progress and provide support.
• Onsite Support: Supervise students in the classroom setting when necessary and assist with the transition back to public school systems.

Required Skills & Experience

• Youth Engagement: Proven experience training, coaching, or guiding youth (experience with at-risk or justice-involved populations is highly preferred).
• Collaborative Mindset: Strong ability to communicate effectively within a multidisciplinary team and create new initiatives.
• Public Speaking: Ability to act as a spokesperson at events and professional meetings.
• Relationship Building: Experience developing partnerships with state, county, and private agencies.
• Administrative Excellence: Skilled in managing detailed documentation and individualized education plans.
