Job Summary:
Tryfacta is seeking a Registered Nurse (RN) for our client in Woodward, Iowa 50276. This is a temporary contract assignment for a State-Operated Specialty Care facility. The Registered Nurse will provide professional nursing care to individuals residing in a residential healthcare setting, including health assessments, medication administration, treatment administration, emergency care, documentation, and coordination with physicians and interdisciplinary care teams. Position Title: Registered Nurse (RN)
Location: Woodward, Iowa 50276
Duration: 6 months
Work Schedule:

  • Morning Shift: 6:00 AM – 2:30 PM
  • Afternoon Shift: 1:45 PM – 10:15 PM
  • Night Shift: 8:00 PM – 6:00 AM
  • Every 3rd weekend is required
    Pay Rate: $50.00–$55.00 per hour

Responsibilities for this position include, but are not limited to:

  • Provide professional nursing care to individuals residing in a state-operated residential healthcare facility.
  • Perform and document physical assessments, quarterly assessments, health evaluations, and medication side-effect assessments.
  • Assess, document, and follow up on illnesses, injuries, incidents, and allegations of abuse according to facility policies and procedures.
  • Safely administer medications, enteral feedings, nebulizer treatments, and other prescribed treatments.
  • Enter, implement, and monitor physician orders in the electronic order entry system.
  • Maintain accurate Medication Administration Records (MAR) and medication documentation.
  • Monitor restraint use and complete required safety assessments.
  • Provide emergency care, including Cardiopulmonary Resuscitation (CPR), First Aid, oxygen administration, and oral suctioning.
  • Prioritize nursing assignments based on the severity of individuals'' health conditions.
  • Communicate and collaborate with physicians, interdisciplinary team members, individuals served, and caregivers.
  • Provide training, supervision, and competency-based instruction to direct-care staff and medication aides.
  • Complete follow-up documentation and coordinate healthcare concerns in accordance with facility policies.
  • Follow universal precautions and infection-control practices at all times.
  • Perform other related nursing duties as assigned.

To be considered for this position, you should have:

  • Current Registered Nurse (RN) License issued by the Iowa Board of Nursing.
  • Current Healthcare Provider Cardiopulmonary Resuscitation (CPR) Certification.
  • Professional nursing experience in a healthcare, Long-Term Care, Skilled Nursing, Residential Care, Developmental Disabilities, Behavioral Health, or similar setting.
  • Knowledge and hands-on experience with medication administration and Medication Administration Records (MAR).
  • Experience performing patient/resident assessments and maintaining accurate nursing documentation.
  • Knowledge of nursing care planning and physician orders.
  • Ability to provide emergency medical care, including CPR and First Aid.
  • Strong communication, teamwork, and interpersonal skills.
  • Ability to train and supervise direct-care staff and medication aides.
  • Ability to successfully complete all required facility training.
  • Ability to successfully pass a Level 3 Background Check as a condition of employment.
  • Ability to work assigned morning, afternoon, or night shifts, including weekends, overtime, and alternative schedules as needed.
  • Ability to work every 3rd weekend.
  • Experience in Developmental Disabilities, Residential Care, Long-Term Care, Skilled Nursing, Group Homes, or State-Operated Facilities is strongly preferred.
