Summary

Job Title: Physical Therapist (Long Term Care)
Location: Haydenville Rd, Leeds, MA 01053
Duration: 13 weeks (with possibilities of permanent hire)

Shift: M-F, 8 hr days-
Guaranteed Hours: 35

Position Summary:
We are seeking a dedicated Physical Therapist (PT) to provide high-quality care in a Long Term
Care setting. The PT will evaluate and treat residents to improve mobility, restore function, and
enhance overall quality of life.
Key Responsibilities:
• Evaluate residents and develop individualized treatment plans
• Implement therapeutic interventions to improve strength, mobility, and function
• Monitor progress and adjust care plans as needed
• Collaborate with interdisciplinary teams to ensure comprehensive care
• Educate residents and families on therapy goals and techniques
Qualifications:
• Active Massachusetts Physical Therapist license
• Experience in Long Term Care of 1 Year
• BLS Certification
• Strong clinical, communication, and documentation skills
