This person will report into - The Physician Relationship Manager (PRM) - Data & Analytics supports growth, care continuity, and network integrity through advanced analytics, reporting, and CRM optimization. This role translates claims, EHR, and provider performance data into actionable insights that improve targeting precision, outreach effectiveness, care continuity, and patient conversion. This position serves as the analytical partner to Physician Relationship Managers, Ambulatory Operations, and Service Line leaders by identifying growth opportunities, monitoring performance trends, and strengthening data governance across CRM, referral systems, and provider-facing tools (e.g., electronic referral Job Description Primary Responsibilities Data ingestion, validation, and governance Manage referral and provider data feeds, enhancements, and system improvements Combine and clean data across multiple sources (e.g., claims data, EHR data) Partner with IT and enterprise analytics teams to ensure data accuracy and usability Integrate multiple data sources into a consistent, governed dataset that feeds dashboards and reports, and supports measurement of intended outcomes Analytics and insights Analyze referral patterns, care continuity data, provider performance, and service line trends to identify growth and outreach opportunities Detect out-of-network referrals and care continuity gaps; prioritize high-impact intervention areas Develop provider segmentation and targeting strategies in partnership with PRMs Synthesize findings into executive-ready summaries with clear recommendations Identify referral-to-encounter workflow breakdowns and escalate systemic barriers Performance reporting and dashboards Design, maintain, and enhance dashboards translating referral data into operationally actionable insights Deliver routine and ad hoc performance reports, including in-network and out-of-network referral rates, conversion, and share of care Track intervention outcomes and quantify impact over time Facilitate recurring care continuity performance retrospectives CRM Maintain CRM data structure, reporting integrity, and documentation standards Train and onboard PRMs to CRM best practices and reinforce documentation compliance Cross functional collaboration Onboard providers to electronic referral pathways tools Partner with Ambulatory Operations, Acute Operations, Access, and Service Line leaders Serve as liaison between frontline PRMs and enterprise analytics teams Support development of playbooks and structured interventions to improve referral integrity

Pay and Benefits
