Job Description

We are looking for a Health Center Quality and Practice Support Advisor to support and coach participating health centers in improving chronic disease prevention, management, and care coordination through quality improvement initiatives, including the Indiana Primary Care Learning Collaborative (IPCLC) and the Promoting Integration of Primary Behavioral Health Care (PIPBHC) program. This position is a full?time contract role and is hybrid (3 days onsite in downtown Indianapolis, 2 days remote).

What You?ll Do

? Support Community Health Centers, FQHCs, and look?alikes participating in the Indiana Primary Care Learning Collaborative
? Coordinate and facilitate quality improvement work related to chronic disease management, prevention, and care delivery
? Provide coaching and facilitation using quality improvement methodologies such as the Chronic Care Model (CCM), Patient?Centered Medical Home (PCMH), Plan?Do?Study?Act (PDSA), and the Model for Improvement (training provided)
? Facilitate interdisciplinary team development within participating health centers
? Support clinicians and medical staff in implementing and spreading quality improvement concepts across practices and teams
? Ensure best practices, lessons learned, and successful strategies are shared across participating health systems
? Assist health centers with integrating learning aims and change strategies into broader community and behavioral health initiatives
? Foster partnerships between IDOH, health systems, behavioral health clinics, local health departments, and community organizations
? Provide on?site technical assistance and serve as a liaison between participating health systems, technical support resources, and IDOH leadership
? Collaborate with health system leadership, including CEOs, and external partners such as the Indiana Primary Healthcare Association (IPHCA)
? Support data tracking, indicator measurement, and submission through an online reporting system
? Work with IDOH leadership to coordinate measurement, analysis, and reporting of quality improvement progress
? Support quality improvement teams between learning sessions to strengthen data?driven decision?making, care coordination, population health management, and operational efficiency

What You?ll Need

Required:
? Medical degree or bachelor?s degree in health science, health policy, health administration, social science, or a related field
? Knowledge of health systems and chronic disease management models
? Experience working in or with primary care, ambulatory care, or community health settings
? Strong communication, facilitation, and coaching skills
? Ability to work independently and collaboratively across multi?site, interdisciplinary teams
? Proficiency with computers, data reporting tools, and clinical information systems

Preferred:
? Clinical background in primary care or community health
? Familiarity with the Chronic Disease Prevention and Management Framework
? Experience with quality improvement methodologies and collaborative learning models
? Coursework or training in leadership, facilitation, adult education, or project management
? Experience supporting health centers or public health initiatives

Physical Demands

? Ability to safely and successfully perform the essential job functions consistent with the ADA and other federal, state, and local standards
? Sedentary work involving sitting or remaining stationary most of the time, with occasional movement required for meetings and travel
? Ability to conduct repetitive tasks on a computer, utilizing a mouse, keyboard, and monitor

$40 per hour

The pay range for this position is listed above.Exact compensation may vary based on several factors, including location, experience, and education.
