Job Overview:

We are seeking a senior-level Medicaid transformation leader to support the South Dakota Department of Social Services (DSS) in overseeing Rural Health Transformation (RHT) initiatives funded by the Centers for Medicare & Medicaid Services (CMS). This role will provide enterprise-level program management across multiple interconnected Medicaid initiatives, including Primary Accountable Care Transformation (PACT), Rural Health Access and Quality Grants, and Behavioral Health transformation efforts. The ideal candidate brings deep Medicaid expertise, experience managing CMS-funded initiatives, and a strong background in healthcare transformation, grant compliance, and multi-stakeholder coordination.

Responsibilities:

Enterprise Program Leadership

  • Provide centralized program management across multiple RHT-funded initiatives
  • Develop and maintain integrated workplans, timelines, and milestone tracking
  • Coordinate cross-agency and cross-program efforts
  • Track risks, dependencies, and mitigation strategies
  • Prepare executive-level status reports and briefings

CMS Grant Compliance & Reporting

  • Support compliance with CMS grant requirements and federal reporting expectations
  • Assist with the preparation of quarterly and annual reports
  • Ensure alignment with federal evaluation frameworks
  • Monitor subrecipient performance and address non-compliance issues

Medicaid Primary Accountable Care Transformation (PACT)

  • Support the design and refinement of population-based alternative payment models
  • Assist in the development of quality and cost performance measures
  • Facilitate stakeholder engagement (providers, payers, state leadership)
  • Coordinate with data vendors supporting performance measurement

Rural Health Grant Oversight

  • Support grant administration processes
  • Monitor grantee performance, deliverables, and financial compliance
  • Provide technical assistance to funded entities
  • Track budgets and expenditures in coordination with finance teams

Stakeholder & Interagency Coordination

  • Facilitate stakeholder meetings and workgroups
  • Coordinate with the Department of Health and the Governor’s Office
  • Support communications and workshop planning
  • Serve as liaison between DSS leadership and external partners

Required Qualifications:

  • Bachelor’s degree (Master’s in Public Health, Public Policy, Healthcare Administration, or related field preferred)
  • 7+ years of experience in healthcare or Medicaid program management
  • Direct experience working with Medicaid programs (State Medicaid agency experience strongly preferred)
  • Experience managing CMS-funded grants or federal healthcare initiatives
  • Experience supporting alternative payment models (APMs) or value-based care initiatives
  • Demonstrated experience coordinating multi-vendor or multi-stakeholder initiatives
  • Strong facilitation, communication, and executive reporting skills

Preferred Qualifications:

  • PMP or Certified Change Management Professional (CCMP)
  • Experience with HEDIS or quality measurement frameworks
  • Experience in rural health transformation initiatives
  • Behavioral health policy familiarity
  • Prior experience working with South Dakota or Midwestern state Medicaid programs
