Remote Director of Telephonic Care Management

RESPONSIBILITIES:
Kforce has a client in need of a Remote Director of Telephonic Care Management.

Responsibilities:
* Lead and expand care coordination, engagement, and support programs to improve participant outcomes and experience
* Develop and execute strategies that increase program participation, retention, and overall engagement
* Provide leadership and direction to an interdisciplinary organization, including oversight of an indirect team of 25-50 professionals across multiple functions
* Partner with internal and external stakeholders to enhance care delivery models and service offerings
* Develop talent strategies focused on team development, retention, training, and organizational growth
* Use data, analytics, and performance metrics to optimize programs and identify opportunities for improvement
* Oversee program implementation, process improvements, and operational effectiveness
* Support the adoption of new technologies, digital tools, and innovative approaches to care and engagement
* Drive continuous improvement initiatives focused on quality, efficiency, and participant satisfaction

REQUIREMENTS:
* Bachelor's degree in Healthcare, Public Health, Social Services, Business, or a related field
* 7+ years of experience in healthcare programs, care coordination, patient engagement, or related services
* 3+ years of leadership experience managing teams, large-scale programs, or operational initiatives
* Experience leading through managers and influencing cross-functional teams in a matrixed environment
* Demonstrated success overseeing and developing large teams, including indirect leadership of 25-50 team members
* Experience developing, scaling, or improving healthcare or care management programs
* Strong knowledge of participant engagement, retention, and service delivery strategies
* Proven ability to drive organizational change and operational excellence
* Experience using data, analytics, and performance metrics to support decision-making
* Strong project management, communication, and stakeholder management skills
* Experience with value-based care, managed care, caregiver support programs, or care management certifications is a plus

The pay range is the lowest to highest compensation we reasonably in good faith believe we would pay at posting for this role. We may ultimately pay more or less than this range. Employee pay is based on factors like relevant education, qualifications, certifications, experience, skills, seniority, location, performance, union contract and business needs. This range may be modified in the future.

We offer comprehensive benefits including medical/dental/vision insurance, HSA, FSA, 401(k), and life, disability & ADD insurance to eligible employees. Salaried personnel receive paid time off. Hourly employees are not eligible for paid time off unless required by law. Hourly employees on a Service Contract Act project are eligible for paid sick leave.
